The diagnostic safety toolkit provides strategies to make improvements through communication. 

Child Health PSO defines communication failures as verbal and non-verbal communication that should have occurred and didn't, or communication that occurred but wasn't effective. Examples include poor documentation, misinterpretation, incorrect assumption, and incomplete, wrong or omitted information.

Once you complete the survey, you will have access to the resources within the five components that make up the patient safety toolkit.
