This presentation will examine the alignment of population health strategies with more traditional missions of tertiary pediatric health systems. Using the experience of a growing program at the Children’s Hospital of Philadelphia, the presentation will aim to:
- Characterize the challenges around aligning strategies for population health in large pediatric health system.
- Demonstrate how the steps of a population health model can align with quality improvement methods to demonstrate measurable result.
- Propose opportunities to align population health strategies with overall institutional goals in large pediatric health system.
