As you prepare to submit a proposal for a presentation or poster, use this information to ensure you have gathered all the necessary information to submit.
Authors may stop and start editing proposals or save and return later any time prior to the submission deadline. Consider developing your proposal in collaboration with your organization's QI team. Also consider seeking input from your organization's communications team for editing before submitting.
The Conference Advisory Committee reviews all submissions and quickly filters out poorly written and vague submissions.
1. Review getting started information, including:
- General directions
- Deadlines and notifications
- Presenter agreements (including registration requirements)
- Proposal outline and guidelines
2. Create a new user account
The information provided should be a true and accurate description of the lead presenter.

3. Presenter questions and agreements
- Confirm information is correct and accurate.
- Review and agree to the Presenter Agreements.
- Have you (or others) presented this information in the past 12 months at another conference?
- List the conference or meetings where this information has been presented.
- You will be asked to read and accept the presenter agreements in order to submit your proposal.
4. Proposal information
Information provided in this section must be blinded (no mention of hospital, presenters, city or any other descriptors that may provide reviewers with an idea of who or which children’s hospital submitted the proposal). Abstract information will be used to describe your presentation and what attendees will learn and will be included in conference marketing materials and website.

Learning Objective 1 (limit 20 words)
Learning objectives must describe what participants will be able to do as a result of attending the session. Objectives must be stated as observable behaviors, completing the sentence, "After completing this activity, participants will be able to..."
Verbs denoting mental states such as "know," "understand," or "appreciate" should be avoided. Instead, use action verbs such as "describe," "discuss," or "explain." View a listing of measurable verbs.

- State the problem that you were solving for.
- Include intended target state or measure of success.
- Describe how you advanced work solutions in the conference focus area selected.
- Provide a brief contextual background of the situation and description of the project and details on how you advanced project aims.
- This description will be shared with conference attendees.
Project Outline or Framework (limit 100 words)
- Provide details on how your team advanced project aims.
- Describe how you advanced work solutions in the conference focus area selected.
- Include interventions that were implemented to achieve your target state.
- Include data used to validate the problem.
Transferable and Replicable (limit 75 words)
Please share any attempts to replicate project within your hospital or health system explain (if applicable). Alternatively, please tell us why you consider this initiative relevant and transferable to peer children’s hospitals.
Results, Outcomes and Sustainability (limit 75 words)
Please detail clear projected or achieved results, outcomes, and/or sustainability efforts. Describe strategies for maintaining gains or improvement. Share barriers to sustainability and any tactical tips for holding performance.
Lessons Learned (limit 50 words)
Describe what your team learned from this initiative or project—successes and failures. Describe what would you do differently based on lessons learned.
Future of Initiative/Next Steps (limit 50 words)
Describe next steps for this project.
Health Care Team (limit 50 words)
Describe how this initiative, project, or topic engages a team of health providers and/or patients for collaborative and coordinated approach to shared decision making. If you are using any CHA programs in this initiative, please indicate the program below.
5. Co-presenter(s) information
Identify any co-presenters. The call for proposals online application allows for two presenters per proposal. The recommended number of presenters for a standard concurrent session and poster presentation is two. Presenter adjustments may be made after the call for proposals review process.
Non-member co-presenters must be disclosed during the call for proposals process. If your additional presenter is not a direct employee of the children's hospital, please indicate their organization accurately. You can add a non-member to the system using the "new registration" button in the search options.
