Value-Based Care

The transition to value-based care (VBC) marks a substantial change in care delivery and payment models that support effective population health management. New models designed to improve children's short- and long-term health are increasingly possible as payment models migrate away from fee-for-service contracts.

Assembled in 2017, the Accountable Health Learning Collaborative (AHLC) is a venue for children's hospitals to share and learn as systems, relationships and capabilities evolve to assume increasing accountability for populations of children. The participating organizations represent varying operational settings and community demographics, but share a common goal of building appropriate strategy for the pace of change towards value-based care in their markets.

Navigate the intricacies of moving from fee-for-service to value-based care using resources based on the AHLC's experiences.
