When patients seek medical care, they often are facing critical challenges in their lives – little food, lack of a job or a struggle to keep up with utility bills. Unfortunately, these challenges often affect their health. The Family Resource Connection at Dayton Children’s Hospital uses a doctor’s visit as an opportunity to address non-medical issues that impact health. This program screens patient families for social needs and offers resource connection and follow-up. With a year of program implementation and nearly 1000 families enrolled in the program, Dayton Children's is now reviewing our data and implementing a number of PDSA cycles to improve our process, the skills of our student workforce, and address some of the challenges we have identified. We have identified specific processes that can be replicated to scale to new audiences.
