Population Health Survey

Overview

Fielded in 2015, the population health survey was designed to bring value to all CHA members through timely, relevant benchmarking data about the degree to which children’s hospitals are involved in population health.

This survey includes questions about:

  • Population health engagement
  • Structure
  • Staffing
  • Measures
  • Payment and funding
  • Data accessibility
  • Community health needs assessment
  • Partnerships

Survey advisory group

In addition to a variety of CHA staff, seven members representing diversity in hospital type, geography and professional role acted as the Survey Advisory Group to influence content and test the initial survey. CHA thanks the advisory group for their participation.

Eligibility and enrollment
