Saving Our Smallest: A Community Approach to Reducing Infant Mortality

This webinar will share best practices and lessons learned  from ProMedica and Northwest Ohio Pathways HUB partnership to reduce infant mortality. Using evidence-based screening questions to identify high-risk patients, and then connecting them to services, this community wide collaboration has begun to make a significant difference in the health outcomes of our patients.
