CHA is offering a series of member highlight webinars to bring the concepts presented in the 2017 Essentials in Population Health series to life through member hospital experiences.
Join us for this member highlight webinar where two hospitals will share the steps they've taken to engage the community and improve patient and population health outcomes.
Boston Children's Hospital started with a specific patient population - those with Rett Syndrome - and worked to identify high priority gaps in care and opportunities to improve coordination. Based on the input of from family partners, the hospital developed and implemented a set of care integration processes and measures aiming to improve care coordination and integration. After one year of the intervention, a measurable drop in charges was recorded as was a measurable increase in patient/family satisfaction. The tools and processes are not being expanded to other patient populations.
At HSHS St. John Children's Hospital in Springfield, Illinois they've taken a program that grew out of a local County Health needs Assessment to address access to care in a blighted neighborhood and developed something targeted to address the needs of children in the neighborhood, many who are medically complex and/or socially fragile. The presenters will share the background of this innovative program that has resulted in coordinated care services for children with complex needs that are growing up in families struggling with many social determinants of health (SDOH).
